Before Boarding is sharing on its Instagram profile [@BeforeBoarding] and its app the digital form required by multiple institutions for entering and leaving the Dominican Republic. In case you weren’t aware, as of April 1, 2021, paper forms are no longer valid for these purposes.

In accordance with laws 285-04, 115-17, 72-02 and 226-06, it is mandatory for each passenger to complete accurately the information requested on the electronic ticket. This information is required by the General Directorate of Migration, the General Directorate of Customs and the Ministry of Public Health.

The good news is that this single form replaces the three (3) forms previously used: International Embarkation and Disembarkation Card, Customs Declaration Form, and Traveler’s Health Affidavit. 

It is important that if you are traveling as a family, you fill out the form with the information of all the members who will be traveling. Only one (1) user needs to be created in the portal and this user would be responsible for completing the data for all the members in question.
